Requirements for Minors:

Ages 5-11 need 3 things to get vaccinated:

Consent form signed by parent/ guardian

Accompanied by an adult (parent/guardian/other)

Parent/guardian signee has to be available via phone if not in person to confirm.

Ages 12+ need 3 things to get vaccinated:

Consent form signed by parent/ guardian

No guardian needs to be present

If they have their vaccination card from their 1st dose they do not need a consent form for their 2nd dose. 1. 2. 3.

Who can get vaccinated? Vaccine eligibility in Pima County is open to everyone age 5 and older

How many doses will I need? Both Pfizer and Moderna vaccines are two-dose vaccines, and the Johnson & Johnson vaccine is a single dose.

Who is eligible for the COVID- 19 booster shot?

 

The CDC recommends booster vaccines for anyone 16 and older six months after their second dose of the Pfizer or Moderna vaccines, or two months after their single shot of Johnson & Johnson.
